Medical Records and Your Privacy
DePauw Health Wellness Center utilizes the EPIC electronic medical records (EMR) system to enable better coordination of care, faster test results, and more streamlined physician ordering. The EPIC EMR system has a patient portal called MyChart to access medical records, test results and appointment summaries. Make sure to speak with your Medical Assistant about getting registered for MyChart in order to access your medical records.
Individuals who have been seen in the DePauw Health Wellness Center can access their medical records through MyChart or request their medical records by completing this form and returning it to the DePauw Health Wellness Center.
