Optional Practical Training (OPT) Update Form

As your OPT sponsoring institution, DePauw University is required to regularly report your employment, residential and current contact information to SEVIS (Student and Exchange Visitor Information System). You as an individual in F1 immigration status currently on OPT are required to regularly keep your parent institution (DePauw University) updated about your employment details and contact information.
