WORKER'S COMPENSATION
As outlined in our Employee Handbook, any employee injured while working for the University, either on or off campus is expected to fill out and submit to the Office of Human Resources an Indiana First Report of Injury Form within 48 hours of the incident. Please fill out section "Employee Information" and "Occurrence/Treatment Information on this Indiana form and return to the Human Resources Office via campus mail or drop off at the office on the first floor of the Administration Building.
